Many employers have matching gift programs and will match charitable contributions made by their employees. Matching gifts can have a great impact in your fundraising-- doubling or possibly tripling a donation! To find out if your company has a matching gift policy, please enter your employer's name below.

Information for Donors:
If your company has a matching gift program,  please submit your match using your company's form. If you don't know where to find it, we suggest asking your Human Resources department. Once your company has confirmed that they will match your donation, please send confirmation email/screenshot to the CHOC Foundation at corporate@choc.org and the participant you're supporting.

For your reference, the CHOC Foundation Tax ID Number is: 95-6097416

Information for Participants:
To receive credit for your matching gift donations as part of your CHOCstruction fundraising total, please ask each of your donors to forward you his/her matching gift receipt, and then follow these instructions to receive credit for the match. 

Forward the email confirmation of the matching gift to corporate@choc.org. Make sure the following information is included in the email:
a. Donor’s name & original donation amount
b. Participant's name
c. Company name & dollar amount of the match
